New patients must have a referral from their primary care provider (PCP) or the physician managing their ongoing care before scheduling an appointment. Please ask your provider to fax the referral to our office at (775) 358-1314.
Cancellation/ No Show Policy for Appointments
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment book. If an appointment is not cancelled at least 48 hours in advance you will be charged an $80 fee.
During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. We accept most insurances.
Patient Forms
Please print and fill out the New Patient forms so we can expedite your first visit.
New PatientsIf you are unable to bring your completed new patient forms with you to your appointment and need to fill them out in our office, you must arrive 30 minutes prior to your scheduled appointment time or we will need to reschedule you. Also, please bring the following:
- Patient's insurance card
- Picture ID (drivers license)
- List of current prescription and/or over-the-counter medications, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have any questions about whether we accept your insurance, please call your insurance company to confirm if we are in-network. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
